Guthrie Cards:

For Blood Spots:

We will provide our clients with Guthrie cards for blood spot collection. Each of the cards will be bar-coded with an accession number unique to your site. We have two types of cards, one for sick infants and another for newborn screening. We require our clients to use the appropriate cards and not to pass cards from site to another as this lead to confusion when we send our reports. The bar-coded cards should not be used for urine or CSF samples. Please see instructions on how to fill the blood spots (Instructions for blood spot collection).

For Urine Spots:

We will provide our clients with special filter paper for urine sample collection. These should be only used for urine. (Instructions for urine spot collection).

Requisition Form:

We will provide samples of our requisition form, which can be photocopied and used or can be printed from this WWW site (To Print Requisition Form). We do request a separate requisition form for each of the patient’s samples and for each type of test.

As a minimum, the patient’s name should be written clearly on the specimen tube and penciled on blood spots and urine spots.

It is of utmost importance to fill the requisition form accurately and provide the necessary patient name, birthdate, and the clinical history section.
